What to Expect
After you schedule an initial appointment with Dr. Singleton, you will receive a link in your email to complete the intake paperwork and electronically sign the informed consent, privacy, and office policies documents through a secure online portal. It is important that you complete this paperwork before attending your appointment.
The following fees are self-pay rates for services. If I am an in-network provider for your insurance plan, rates are determined per contract for each plan. If your insurance company does not pay the full amount of this rate based on your coverage benefits, you will be responsible for the remainder of the rate.
45 minute therapy session (38-52 minutes): $130
60 minute therapy session (53 - 60 minutes): $150
60 minute initial intake session (53-60 minutes): $165
Psychological/Psychoeducational Testing: $150 per hour for administration, test scoring/interpretation, and evaluation report writing.
Career/Vocational Assessment: $150 per hour for administration, test scoring/interpretation, and evaluation report writing.
*Payment for fee-for-service/self-pay evaluations is due in full at the end of the first testing session in order to schedule remaining testing appointment(s). You must notify Dr. Singleton of any cancellations within 48 hours prior to your testing appointment, otherwise you will be charged the cancellation fee.
Dr. Singleton is an in-network provider for the following insurance providers:
Blue Cross/Florida Blue/New Directions
United Health Care (not Golden Rule)
Before your first appointment it is very important that you understand what your insurance covers. Your sessions may be covered in full or in part based on your coverage.
Please call your insurance provider and ask the following questions:
Do I have coverage for mental health services by a psychologist?
What is my copay?
Do I have a deductible and has it been met?
How many sessions are covered per year?
What is the coverage amount for a 45 minute or 60 minute therapy session?
Is a preauthorization required before I can see a psychologist?
Do I need a referral from a primary care physician first?
Dr. Singleton is an out-of-network provider for all other insurance plans. It is possible that if you have out-of-network benefits you can have part of your session reimbursed to you by your insurance provider; however, you will be responsible to pay the full fee at time of service. Dr. Singleton can give you a superbill after your session to submit to your insurance provider.
If you do not attend your scheduled appointment and do no notify the office 24 hours in advance (48 hours for psychological evaluations), you may be charged a fee as outlined in our office policies.
